Our Ocean Forms
How it works:
Before your appointment, we will send an Ocean form to your email, which you can fill out with all your relevant information. (The email will be sent from
no-reply@cognisantmd.com)Then, our volunteers will add everything to your file and double-check the information with you during your next visit.
Dr. Curnew will also see all of this information, meaning you can bring up concerns you may forget to mention during your appointment.
Being able to cross-check information means Dr. Curnew can better serve your healthcare needs and reduce the chance of making a medical error!
Ocean is a new initiative run by Dr. Curnew that helps our clinic obtain more accurate information, and allows you the opportunity to mention things that may not come up during your appointment!
